The Efficats of Combination Therapy with Clarithromycin and Omeprazole in the Eradication of Helicobacter Pylori for Treatment of Patients with Doudenal Ulcers



Abstract
Although it is presently recognized that Helicobacter pylori (H. pylori) infection is the main acquired factor in the pathogenesis of duodenal ulcer disease and that duodenal ulcer patients must be treated with antimicrobials in order to greatly
reduce,
or even abolish, the risk of recurrence, a consensus aggrement for the optimal therapy has yet to be accomplished. But, the best treatment for H. pylori is still unsettled. Recent studies have shown that clarithromycin, particulaly in combination
with
omeprazole has high efficacy in the eradication of H. pylori. The aim of this study was to evaluate the efficacy of combination therapy with clarithromycin and omeprazole, and was compared to that of amoxacillin and omeprazole for treatment of
patients
with duodenal ulcer associated with H. pylori.
@ES Methods:
@EN Subjects with H. pylori positive duodenal ulcer, who had not received previous eradication therapy were enrolled. Subjects were H. pylori positive if two of the following were positive: antrum histology and rapid urease test (CLO test).
Subjects
were H. pylori negative if all tests were negative. Subjects were allocated to one of the following: the group A received the combination therapy with clarithromycin(1,500mg/day) and omeprazole (40mg/day), the group B with amoxacillin
(1,500mg/day)
and
omeprazole(40mg/day). The eradication treatement was 14 days duration in both groups. All the patients underwent a gastroscopy with biopsies one month after treatment completion in order to assess the presence or absence of H. pylori.
@ES Result:
@EN The therapeutic response rate in eradication of H. pylori was statistically different between in group A(80%, 12/15) AND b(56.6%, 8/15), but there was no differences in the rates of improvement when compared their symptoms after treatment in
both
group. One patient in group A dropped out because of adverse symptoms and 29 patients completed the study.
@ES Conclusion:
@EN The combination therapy with clarithromycin and omeprazole has clear advantages over those with amoxacillin and omeprazole forthe eradication of H. pylori in patients with duodenal ulcer associated with H. pylori positive. These results show
that
combination of omeprazole with clarithromycin can be recommended for H. pylori eradication.
